Authorization to Receive Customer Information

Effective January 1, 2024, PSEG Long Island will be implementing a new process for the Letter of Authorization (LOA). In order to obtain customer information, the form must be filled out in its entirety. All previous LOA’s will no longer be accepted. 

Before providing requested customer information, all areas on the form must be filled out clearly and signed. Any incomplete sections, will be returned. All completed LOA’s will be verified and confirmed by the customer.
